How will my personal information be shared?
LSLCS services are very confidential: we only share things you tell us with people outside the service with your permission, unless we need to safeguard a child or young person or a vulnerable adult or you. Leeds OASIS is a partnership between LSLCS, Touchstone and the NHS (Leeds and York Partnership Foundation Trust). If you are in Leeds OASIS, you will also have an NHS worker from CRISS or IHTT assigned to look after your care whilst you are with us. Information sharing between the partner organisations of Leeds Oasis will feel a little different. Please talk with us about this if this causes you any concerns.
We will be liaising with NHS workers on a daily basis to make sure that:
- We are working alongside our colleagues in the NHS in a “joined-up” way, with all of us having the necessary information to ensure you are getting the right support
- You don’t have to “tell your story” multiple times
Additionally, the NHS now use a “formulation” model. A formulation is a description of your difficulties (and your strengths), written in partnership with you. This can be added to with workers to support you, to make more sense of what’s happening for you. The things you tell us can be used to create a more useful formulation. This will also help in developing a safety plan for you when you leave OASIS.
If you’ve used other LSLCS services, we won’t tell partners in Leeds OASIS details of anything you’ve disclosed at those times.
